Nikolas Mikhail Stavrosovich Cassadine is the son of Laura Spencer & Stavros Cassadine. He was raised by his uncle Stefan and aunt Alexis Davis after the death of his father Stavros and the incapacitation of his evil grandmother Helena. His mother Laura had left him in the care of Stefan when she returned to Port Charles and to her husband Luke. Laura did not see him again until Nikolas was a teen. Luke and Laura's young daughter Lulu was in need of a bone marrow transplant. All seemed lost. Laura then made the decision to call Stefan and ask for help. Nikolas and Stefan arrived in Port Charles. Nikolas was a match for Lulu and the transplant was performed, saving her life. However, Laura's secret was revealed to her family, causing chaos within her family. Luke gave Laura an ultimatum to choose between Nikolas and Luke/Lucky/Lulu. Laura chose to stay with Luke, further hurting Nikolas, who had already felt abandoned by her.

After some time, Luke accepted Nikolas' existence although Luke continues to loudly announce his annoyance with his stepson. Nikolas' relationship his siblings was much better. His relationships with Lulu and Lucky are strong, as are his relationships with grandmother Lesley and his former aunt Bobbie Spencer (from her brief marriage to Stefan). Along with Elizabeth Webber and Emily Quartermaine, Nikolas and Lucky became The Four Musketeers, sharing experiences and adventures that solidified the quartets still-strong friendship. The quartet has survived Elizabeth's rape, Lucky's "death", Emily's paralysis and cancer, Nikolas' amnesia and Nikolas & Emily's divorce, following her rape and his infidelity.

Nikolas recently discovered he and Courtney Jacks were the parents of John Jacks, who had been believed to be the son of Courtney and her husband Jax. Old friend Robin informed Nikolas of the truth at Baby John's christening causing chaos. Courtney's best friend Carly Corinthos and Jax had been raising the son as his. Upon advisement of his aunt Alexis, Nikolas did not press charges but almost immediately took custody of his son. He is currently trying to learn how to be a father with the help of his family and friends. His grandmother Helena has been lurking but Nikolas is determined to keep his son safe.
